Which Authority Handles My Document?
Different documents are controlled by different authorities, and the body that can fix your problem isn't always the one you'd expect:
- University degree, transcript, provisional certificate: Your issuing university
- Matric/Intermediate certificate, marksheet: Your issuing Board (BISE/BTE), not HEC
- Attestation of degrees/transcripts for use abroad: HEC (Higher Education Commission)
- Attestation of Matric/Intermediate certificates: IBCC (Inter Boards Coordination Commission)
- CNIC name, father's/husband's name, date of birth: NADRA
- Nursing/midwifery/LHV registration records: PNMC
- Allied health professional registration records: AHPC
- International legalization / Apostille of attested documents: MOFA (Ministry of Foreign Affairs)
The rule of thumb: the body that originally issued the document is almost always the one that has to fix or reissue it. HEC, IBCC, and MOFA generally attest and legalize documents — they don't create or correct the underlying record.

Lost Degree? Here's the General Path
- Confirm what's actually missing (degree, transcript, or both).
- Contact your issuing university — they are the only body that can issue a duplicate.
- Prepare an affidavit confirming the loss (a standard requirement at university and HEC levels).
- Receive the duplicate (typically marked "Duplicate" on its face as standard practice).
- Proceed with verification and attestation via HEC once the duplicate is issued.
- Complete MOFA attestation or Apostille if the document is needed abroad.
Name Correction: What to Expect
- Minor Correction (Spelling errors, document alignment): Handled through affidavits and supporting evidence (CNIC, birth certificate, academic records) submitted to the relevant authority or NADRA.
- Major Correction (Substantial name change): May require formal legal processes, such as a court petition, before the authority can alter official records.
